The Tzar Bomba was 26×6 feet, meaning it fits in a container or in the back of a regular truck. One each in LA and NY Harbors would pretty much wipe out the entire united states for all intents and purposes. It would not be difficult for this attack to take place. That bomb, the biggest one to date, did not include an accelerator, which would have doubled it to 100m. It would break windows in Oregon. Burn much of california to the border, and restore the desert within 60m of the blast.
