Never happen. They believe over the long term they win even if thats many decades. There is no use ending RU civilization in a nucler war that is not for their survival but for their strategic interests.

Too many people, lacking abilty, knowledge, experience, and achievement opine with certainty about matters of great portent determined by people of great knowledge, experience, achievement and ability.

Even those people who can make such decisions are enabled by a vast hierarchical network of people who will not follow order that lead to the eradication of themselves and their families. In the few near risks that we have had it has been these people whose chool heads prevailed. The risk instead is not that the leadership is irrational, but that someone down the chain of responsibility is irrational instead, and starts something out of emotional stress or error.

Even then they can only launce one weapon.
